Signing an involuntary discharge form?

I was fired last week quite suddenly with immediate effect due to a difference of opinion with my employer. I have been employed by this employer on a permanent basis for almost 2 years. I don't agree with this myself. Do I have to sign this form?

Dear, Your employer may not simply dismiss you. A difference of opinion is not a valid reason for dismissal.
