Exempt from work and getting sick

I signed a VSO at the end of December 2020 and am exempt from work until May 1, 2021. In the months since signing, all kinds of things have been going on in my body that indicate a burnout. Do I have to report sick to my employer? I have agreed on a severance payment and this will be paid in May. Will this be affected if I report sick? Will the VSO now be cancelled if I have to report sick?

Yes, I advise you to report sick. You also have reintegration obligations. Usually nothing will change in the vso by a (later) reporting sick, although I do not know the exact conditions of the closed vso.
