What are the best steps I can take to successfully complete my reintegration?

I received a warning during my reintegration. This warning is completely unjustified, based on an email I sent in which I clearly state my boundaries (advice from the company doctor). The employer states in this warning that I do not cooperate with my reintegration, while this is actually the other way around. I have built up documentation that clearly shows that I am doing my best, and that communication with the employer is not going smoothly due to laxity on the part of the employer. To give an example, during my reintegration (now more than 2 months in progress) I have not had a conversation with my manager and my manager does not take my reintegration into account.
