Terminating an annual contract due to illness - What now?

I have a one year contract with the intention of this becoming a permanent contract. My one year contract expires on January 22nd. Unfortunately, cancer has now been diagnosed and I am going on a path to recovery. I am still working, but it will not be long before I can no longer work and have to go on sick leave. I understand that a contract does not need to be extended. Can it not be extended without giving a reason?

This is allowed without giving a reason. However, the employer has a notification obligation at least 1 month before the expiration of the contract to indicate whether the contract will be extended.
