I have been ill for almost a year now, but have been reintegrating since March and am almost back to my full hours. In any case, I have fully resumed for my second year. My manager has never kept a reintegration file on the course of the illness/reintegration and all the activities that need to be undertaken to make returning to work possible. Unfortunately, there have also never been any efforts or discussions from my manager Every six weeks my manager should have discussed the progress with me, this never happened. In one year we had 3 meaningless conversations. My manager has been negative ever since I became ill about whether I would still be able to do the work, while I had not yet had the chance to prove whether or not I could still do it. I had lung cancer, half of my left lung was removed. I am now cancer-free and I am doing well. I quickly picked up my work again and that is also going very well. Now that I am almost fully back at work, I notice that she is starting to look for me in my work. I fear that when I am fully back at work I will get a bad evaluation so that she can still fire me. What can I do about this and what is the best route to take.

You cannot simply be dismissed, there must be a valid reason for this. Moreover, if you return to work from a reintegration project, the employer cannot simply dismiss you.
