Requesting an expert opinion: what to do?

After I informed the employer that I would request an expert opinion from the UWV, the employer completed the first-year evaluation himself (without involving me). I requested this 1st year evaluation to find out what was filled in. Can I sign this 1st year evaluation as accepted? How can I then add my comments? Should these comments be included on the same form?

You can precede your signature and name with the following text: 'Signed for seen, NOT for approval' If you wish, you can add your comments above the signature, but please first state: 'added, my comments after employer completed form:' Alternatively, you may briefly refer to your attached comments and attach them, stating above your signature: 'My comments regarding the form completed by the employer are attached as an appendix'.
